What is the highest density concrete ever used commercially?

Concrete of over 300 pounds per cubic foot has been used for radiation shielding in volume-limited applications (particle accelerators for medical treatment located inside existing structures).

This concrete has very little water, no fines (ash), and uses scrap steel as an aggregate instead of crushed rock.

There are many engineering challenges in mixing and pouring the concrete — it's very heavy, so only a few cubic yards can go in each large truck, and loading aggregate into the batch plant is much slower than with rock aggregate (due to limited front end loader payloads).
